From my understanding, the time during this "limbo" stage or alternate reality is when everyone is dead in the future, regardless of circumstance. Meaning way after all the events happened so there were those that survived on the island and those that escaped. They just didn't go into details what happened with those who survived over the years. Those who died on the island, died right then and there.
Their "spirits" had come up with a place where they could have some sort of "reunion" once everybody's life has passed. That's the limbo/alternate reality place. So in essence, I guess you can say that they met up in heaven or something. You can see hints when Hurley told Ben before going inside the church that Ben had been an awesome number 2 and with which Ben replied Hugo had been a great number 1. This suggests the idea that they both lived on the island for a time.

Hmm I thought he meant they were "dead", metaphorically. I got thrown off with all repetitive mention of "leaving/living" and what Jacob said that they were leading empty lives hence, metaphorically dead which is why the island needed them.
Them being dead all the while seemed like a cop out to Anne Hathaway's Passengers.
They were only dead in the sideways flashbacks. Basically they were saying that they created the sideways world in order for them to all meet up once again once they had all died in whatever timeframe they passed so they could complete their journey together and pass as one group. We got to see Jack die in the end so it did seem a bit confusing. The "Island" timeline was the true time line.
